Two Ways to Register:

1.  Online Registration                                                                                 

PLEASE NOTE:  A Trinity Medical Release Form is REQUIRED for all students attending events at Trinity. If you have already filled out a Medical Release Form for 2011-2012, you do not need to submit another Medical Release Form.  Please fill out the form, sign and mail to Trinity or drop off at the CE Office. Please contact Nan at 714.505.6254 x101 for questions and/or concerns.  (REGISTRATION IS NOT COMPLETE UNTIL MED REALEASE FORM IS RECEIVED.)  

2.  Print the following forms below and droff off or mail to Trinity:

Trinity Kids (Nursery to 6^th Grade)

Registration Form

Medical Release Form: This is REQUIRED for all students attending events at Trinity. You only need to fill out one form per child per year.

Forest Home Camp Medical Form: If you are signing your child up for Forest Home Camp you will need one of these.

Sunday School Registration: Please fill out a "Medical Release Form" and turn it in to your child's Sunday School teacher to enroll him or her in Sunday school.  If we already have a form from the summer, you can simply inform the teacher that your child will be participating in class and needs a name tag.
